The size you select really depends on the size of the spacer you need, not your shoe size. The loop on the Size Large is only slightly bigger than the Medium. It is stretchy -- if it is too tight on the big toe, you just flip it around and wear it on the second toe. This particular spacer is just made to wear between the large toe and second toe.
We've had small women who needed a Size Large because they needed a wider spacer and large men who wore the Medium because there wasn't that big of a gap on between their toes.
Once again, here are the approximate dimensions of the gel pad for each size:
Medium: Overall 1-3/8" x 1" (Gel pad 1" x 1")
Large: Overal 1-5/8" x 1-1/2" (Gel pad 1-1/4" x 1-3/8")

Relieve Toe Pain!
Visco-GEL® Slip-on Toe Spacers™ reduce irritation between toes and absorb pressure and friction. Soft fabric sleeve eases over toe and keeps spacer in place. Can be used with gel spacer on inside or outside of sleeve. Designed to be used between the 1st, 2nd or 3rd toe, depending on fit. Visco-GEL® slowly releases mineral oil and vitamin E to soothe, soften and moisture skin.
Directions: Slip over desired toe and position as needed. Test for a comfortable fit. For maximum benefit, do not wear shoes that interfere with toe freedom.
Care: Hand wash. Air dry completely. Sprinkle with talcum powder if gel becomes tacky.
Warning: if you have diabetes or poor circulation, consult your physician before using. If numbness, discoloration, irritation or additional pain occurs, discontinue use immediately. If symptoms persist, visit a foot specialist.
